in japan, the age at which a person should be in need of care is defined as when he or she can no longer perform activities of daily living (adl). the adl index was developed by the ministry of health, labour and welfare in 2004 to measure adl of older adults (65 years or older). the index classifies adl into seven items: eating, defecation, bathing, walking, moving around inside the house, dressing and hygiene, and toileting. we excluded the item on bathing because most of the respondents were already certified for ltci (96.9%). the index is based on the responses of the index subject’s caregivers. hence, the information on the care level was missing for the majority of people. among the missing values, about 30% of the adl scores (n=4386/13,531) were missing for the respondents who were not certified for ltci. to ensure that the calculation of the adl index is valid, we excluded these missing values from the analysis (n=12,657). the index is divided into six levels of adl care needs: 0 (no care), 1, 2, 3, 4 and 5. the adl index is used by the ltci-certified persons’ social insurance system to determine their care needs. because the adl index is a care need criterion for ltci certification, the adl index is closely related to the care needs of those not yet certified. in this study, the adl index is used as an indicator of the care needs of older adults. the adl index was calculated using the procedure proposed by the ministry of health, labour and welfare in 2004. in this study, we classified the adl score into two levels: 0 (no care) and 1 to 5 (care needs). the number of people who were not certified for ltci was 7286 in 2006. then, for these un-certified people, the care needs were identified by the adl index in each year. the adl index was calculated by multiplying the adl score by the proportion of people who were not certified for ltci in each year (adl score=0 for the year; adl score≠0, adl score=1 for the year; adl score≠0, adl score=2 for the year; adl score≠0, adl score=3 for the year; etc.).
